Obtaining a referral or insurance preauthorization

Most HMO and some PPO plans require a referral from your primary-care physician before you can see a specialist. If this applies to your plan, please have the referral from your primary-care physician sent to our office before your scheduled appointment.

If your insurance plan requires preauthorization for specific treatments and procedures, we can assist you in getting the needed approval. If you are unsure if preauthorization is needed, please call us and we can help.

Co-payments and out-of-pocket insurance costs

Many insurance plans, both HMOs and PPOs, have a patient co-payment that must be collected at the time of service. This is a contractual requirement of insurers. Please be prepared to provide your co-pay at the time of service.

Wasatch Dermatology is required by law to bill the patient for amounts considered to be “Patient Responsibility” by insurance companies, including Medicare. It is always the patient’s responsibility to provide payment for cosmetic services, as these are elective treatments not covered by insurance. We accept most major credit cards.
